Rogetsu Island, an isolated archipelago located south of Honshu.
Ten years ago, during the 'Rogetsu Kagura' festival, five innocent girls were spirited away. Although they were eventually rescued safely, they lost all memories related to the incident.
Two years later, a more terrifying anomaly occurred. The islanders vanished overnight, and the few who were found were mostly dead. Without exception, the dead had their hands covering their faces, their expressions contorted in agony, as if they had witnessed something utterly horrific before death.
As the mystery deepened, the police eventually abandoned the investigation. Since then, the island has been forgotten by the outside world, becoming a desolate, uninhabited wasteland.
Now, two of the girls who were spirited away—Misaki Asou and Madoka Tsukimori—have returned to Rogetsu Island despite warnings. This was prompted by the bizarre successive deaths of their friends, Marie and Tomoe, whose corpses resembled the twisted state of the deceased islanders from years ago.
Seeking the truth, Misaki firmly believes their friends' deaths are connected to their lost memories. Together with Madoka, they cautiously step into the abandoned sanatorium on the island—'Rougetsu Hall'.

Chasing after her to the Aso Memorial Room, Madoka finds the door firmly locked. Using the flashlight to check the opening schedule next to the door, she learns the key is kept in the Management Office on the second floor.

Mustering her courage to go upstairs, she turns right along the corridor and witnesses the ghost nurse open a door and disappear at the end of the next corridor.

Quietly following her to the Management Office, Madoka suddenly hears the ghost nurse's whisper at the window: 'Go back to your room...'. Suppressing her fear and ignoring the chilling warning, Madoka pushes the door and enters the office.

On the desk in front of the right window, she finds a document, Nurse's Memo, revealing that the key is placed on this desk.
Subsequently, she successfully finds the Vine Pattern Key on the messy desk to the left.
Stepping out of the office, an eerie melody suddenly plays from the hospital's PA system—a terrifying tune buried deep in her memory that she desperately wishes not to recall.

Hurrying back to the first floor, she uses the Vine Pattern Key to unlock the Aso Memorial Room. The moment the door opens, a strong ominous presence surges beside her. Turning her head, she is met with a terrifying, twisted 'Blossomed' face.

On a typewriter desk in the corner of the room, Madoka finds the document Dr. Asou's Notes 1, which reveals the hidden power of this mysterious device called the 'Camera Obscura'.
Just as she obtains the Camera Obscura from a small counter in the room, the Wraith Yoriko Sonohara appears with intense malice and obsession.

After barely repelling the wraith using the unfamiliar Camera Obscura, Madoka Tsukimori's sanity is pushed to the brink. Just as she prepares to open the door and flee the suffocating room, she despairs to find that the outside is already surrounded by countless wraiths... Her consciousness is ultimately swallowed entirely by endless darkness.

Madoka Tsukimori and Misaki Asou get separated in the hall. While wandering and searching, Madoka discovers the 'Camera Obscura', capable of capturing the impossible.
Although she uses it to see the terrifying wraiths and tries to fight back, she is eventually overwhelmed by countless spirits, her consciousness fading into the darkness.
