As the investigation deepens, the dark truth of the island residents collectively contracting 'Moonlight Syndrome' due to the forbidden 'Kari-me' ritual surfaces.

But who is the woman in red depicted in that bizarre painting inside the painter YoMitsu Magaki's room, 'Irodorizuki'...?

Investigating the painting on the floor again and taking a photograph captures the view of the courtyard—Forgotten Scene.

On the central table cluttered with art supplies, he discovers the file Magaki's Creative Notes 2.

Exiting through the door beside the bathtub into the corridor, he catches a glimpse of a familiar figure to his left—the Ghost: Yo Haibara Entering the Gallery moving deep into the gallery.

Following him into the gallery, in front of the far wall, he spots the Ghost: Yo Haibara Gazing at the Painting looking intently at the artwork before vanishing completely.

Walking to the far end, he gathers a dropped notebook from the floor: Black Notebook 3, and the Ebony Sheet Key.

While closely examining the portrait of the woman in red and the five performers' artwork on the left, the sound of rolling wheels echoes behind him.
Turning around, a red wheelchair materializes out of thin air, and the vengeful spirit Tsukiko Chigando attacks.

After driving her back, he retrieves the related document Tsukiko Chigando's Letter 3 from the floor.

Composing himself, Choshiro returns to the elevator lobby and ascends to the fourth floor.

The moment he enters the office, a piercing alarm blares through the space.
Stepping inside the counter area and operating the wall panel forces the alarm to shut down, but faint chaotic cries of female patients echo from deep within the wards...

On the desk at the inner corner of the counter, he uncovers a patient report: 4F Isolation Ward Report 4.

From the report, he learns that the female patient isolated on 4F is named Sakuya. Reaching the isolation ward gates, he witnesses a ghost step inside.

Following the spiritual trail into Room 412, 'Hiyuki'. Beneath the bedframe, he uncovers an audio tape: Yo Haibara's Tape Reel and the diary Sakuya's Diary 4.

While reading the files, the spirit Shoji Katagiri suddenly ambushes him.
Simultaneously, Shie Onosaki joins the fray. He must handle this hectic chaotic encounter carefully.

Successfully purifying the spirits rewards a record left behind by an assistant: Assistant's Notes 7.

Preparing to take the elevator back down, the doors slide open only for the vengeful spirits Shie Onosaki and Fuyuko Shiratsuki to charge outward.

After a fierce battle, he extracts a nurse's journal from the dissolving spirits: On-Duty Nurse's Journal 2.

Once safe, he takes the lift down to the second floor.

Stepping out and looking left frames the Ghost: Yo Haibara Standing before the Secret Door. Attempting to investigate the hidden door reveals it is sealed tight and cannot be bypassed from here.

Detouring through the main lobby, he makes the long journey back to Ayako's room, 'Ranzuki'.

Passing through the small corner door into the inner room, pointing his camera toward the headboard (near the pillow area) allows him to capture the Bound Ghost: Ayako (Blooming).

Pushing open the heavy doors into the hidden room. Searching the pockets of the suit jacket hanging on the left wall reveals Black Notebook 4.

Turning to the stacked wooden crates on the right, he uses a small key to open the wooden box atop them, securing Black Notebook 5 and a Sealed Film Reel.

Gathering his items, he returns to the lobby and takes the stairs down to 1F.

Entering the dining room, he operates the projector to play the Sealed Film Reel.

Returning to the lobby and passing through the hallways out into the courtyard. A terrifying aura hits him as the blooming Sakuya ambushes him from the subterranean gate entrance.
With nowhere left to run, he must make a stand. After a grueling victory, he rushes into the tunnel toward B1F.

Proceeding along the dark passage to the water reservoir control room, he discovers a tape reel on a corner shelf: Tape 'Choshiro's Investigation Record 6'.

Advancing further to the switch room, he retrieves On-Duty Nurse's Journal 3 from a rack beside the desk.

While passing the central opening in the floor, he feels a strange gaze; photographing downward captures the hidden Bound Ghost: Woman Under the Floor.

Stepping into the forbidden Imiya Shrine

Entering the Purification Path through the opposite door and using the Ebony Sheet Key at the far end unlocks the heavy gates to the 'Imiya Shrine.'
Past memories instantly flood his mind—the vivid image of the woman in red awakening inside the left cell...

On the offering table before the central shrine, he obtains an ancient document: Imiya Shrine Keeper's Notes.

Approaching the left confinement cell, the spirit of Sayaka suddenly manifests, requesting Choshiro to deliver the mask fragments to Ruka...

After Sayaka speaks and vanishes, he retrieves the Black Mask Fragment (Right Eye) from the mattress.
On the adjacent nightstand lies a brief note: Scribbled Sentences.

Retracing his path back to the courtyard, the blooming Sakuya ambushes him once more.
Avoiding a drawn-out conflict, he sprints across the courtyard and escapes back into the hospital interior to lose her.

Returning to the 1F corridor from the courtyard and heading toward the elevator lobby, the Ghost: Yo Haibara Heading to the Old Building manifests near the connecting passage, as if guiding him.

Following his footsteps through the connecting passage, he enters Haibara Infirmary.

Arriving at the director's office, he uncovers Director Haibara's Notes 3 on the bookshelf right of the main desk.

Leaving the office, he descends the left staircase to B1F, makes his way back to the elevator, and takes it down to B2F.

The moment he sets foot in the Moon-Reflecting Cave, the vengeful spirit of Shigeto Haibara, holding a young Ruka, manifests to block his path. He must defeat him.

Emerging victorious from the battle, he hands the previously acquired mask fragment to the young Ruka, fulfilling Sayaka's entrustment.

At that moment, out of the corner of his eye, he catches sight of Yo Haibara escaping.

Chasing Yo Haibara all the way back to the elevator, he finds the suspect has already taken it up. Choshiro follows immediately in the adjacent car (ascending automatically).

The lift reaches 2F. The moment the doors slide open, he sees the Ghost: Yo Haibara Fleeing from Kirishima frantically running from the left corridor toward the right.

Tracking his flight into Ward 2, he catches another glimpse of the Ghost: Chased Yo Haibara rushing frantically toward the far exit near the center bed.

Chasing him out into the corridor containing the laundry room, he looks up to see the Ghost: Yo Haibara Ascending to the Rooftop escaping up the final staircase.

Bursting onto the rooftop, he finally corners Yo Haibara.
However, as he draws near, a sharp pain pierces his abdomen—Yo ambushes him, driving a blade into his stomach.
Through the intense pain and fading consciousness, Choshiro lunges forward with his remaining strength, dragging Yo Haibara over the edge with him...

Amidst the rushing wind of the fall, the brutal reality finally unveils itself. In truth... Choshiro Kirishima had already died eight years ago.

The one who kidnapped the young girls during the Rogetsu Kagura ten years ago was indeed Yo Haibara.

He and his father, Shigeto, kidnapped five girls to play the critical roles of 'Performers' required to secretly execute the forbidden, ancient subterranean ritual: the Ceremony of the Return.

The central figure acting as the 'Vessel' was Shigeto Haibara's daughter, Sakuya, toward whom Yo harbored a twisted, obsessive affection.

Sakuya was a severe patient of Moonlight Syndrome, and the Haibara father and son believed that only by performing the Ceremony of the Return with the completed Mask of the Lunar Eclipse could she be saved.
However, the ritual failed catastrophically, causing Sakuya to slip into a comatose state between life and death deep beneath the hospital, entering a slumber of unknown duration...

At this moment, Choshiro finally remembers everything clearly.

Two years after the disappearance incident, Sakuya suddenly awoke, wandering across the island and causing the residents to 'bloom' one by one.
That day was the 'Day of Total Blooming.'
Choshiro had tracked Yo Haibara to the island on that exact day and witnessed the horror; in the chaos, he cornered Yo onto the rooftop, ultimately plunging to his death alongside him.
