“Where am I? Why am I here?”

Private investigator Choshiro Kirishima awakens at the main entrance of Haibara Infirmary.

Choshiro recalls being entrusted by Ruka's mother, Sayaka, to go to the island and find the missing Ruka. Ten years ago, when the "spirit-away" incident occurred on the island, he was the detective who found Ruka and the four other girls.

Choshiro, having arrived on the island, awakens from an inexplicable coma. Looking up, a full moon hangs high in the sky. He finds himself at the entrance of Haibara Infirmary and witnesses the police-wanted Yo Haibara walking deep into the infirmary...

Moving forward, Choshiro picks up his dropped items: the Choshiro's Notebook, a Tape Recorder, and the recorded tape Tape 'Choshiro's Investigation Record 1'.

Looking at the familiar scenery, Choshiro recalls years ago, it was he who found Ruka and the four other girls spirited away in the basement of this infirmary.
Among the five rescued back then, two have already died mysteriously. Could it be... that the incident from ten years ago is not over yet?

Immediately upon entering the lobby, Choshiro is viciously attacked by three spirits: Ghost: Katsuto Kariya, Ghost: Yoriko Sonohara, and Ghost: Sanae Houzuki.

Choshiro does not have the Camera Obscura to exorcise spirits, but he is equipped with the Spirit Stone Flashlight, a special weapon entrusted by Sayaka that can use moonlight to purify spirits.

After narrowly defeating the three spirits with the flashlight, he finds the document Wandering Precautions on the reception desk to the right.

Proceeding to the elevator near the stairs, Choshiro recalls that he took this very elevator to the basement years ago to find Ruka and the others...

At that moment, a child's voice echoes from the stairwell. Following the sound to the second floor, he hears movement inside the nurse's station and enters.

On the table to the left of the door, he retrieves the file Medical Record: Asagi Hizuki.

In the bookshelf in the right corner, he obtains the file Assistant's Notes 1, recording the assistant's observations on the unique therapies at Haibara Infirmary.

After investigating, he opens the door to the upper right to enter the general ward.

On the bed in front, he spots the lonely figure of Ghost: Long-haired Girl.

At the bed where the long-haired girl was, he retrieves the fear-filled file Picture Diary 1.

At this moment, the residual image of Ghost: Assistant walking in the corridor appears in the passage behind.

Entering the rear passage and passing through the curtained corridor near a wheelchair, he spots Ghost: Assistant in the Smoking Area at the end.

On the ashtray at the end of the smoking area, Choshiro finds a Flat Iron Key (labeled "Surgery Prep Room") and a Haibara Infirmary Map.

As he turns back to return to the ward, he is ambushed by Ghost: Sanae Houzuki.

After going downstairs, the residual image of Ghost: Assistant heading to the Surgery Room flashes across the corridor leading to the surgery room, seemingly preparing for a dangerous surgery.

Entering the passage where he saw the ghost, he uses the Flat Iron Key to open the door.
Passing through the heavy curtain, he enters the Surgery Prep Room.

Inside the locker to the right, he retrieves the file Magazine 'The Mystery of Mass Disappearance', which mentions the mysterious disease on Rogetsu Island. Then he enters the 'Surgery Room' through the upper right door.

Approaching the central operating table, he sees Ghost: Doctors performing surgery surrounding a patient, seemingly conducting a desperate ritualistic surgery.

After a hard battle to defeat her, he obtains the file Picture Diary 2.
Then, on the operating table, he obtains the file Autopsy Report: Asagi Hizuki. At this moment, Choshiro spots the ghost of the assistant walking into the adjacent record room.

Following into the depths of the record room, he can photograph Ghost: Assistant at the desk.

On the shelf to the right, he retrieves the file About the Rogetsu Kagura. Seeing this flyer, Choshiro suddenly recalls that today is the day of the island's decennial festival...

As he intends to open the door to return to the surgery prep room, the remorseful Ghost: Shoji Katagiri suddenly appears to block his way, initiating a battle.

After winning, he obtains the whistleblower file Spirit-Away Incident Report.
On the way back to the lobby, he is caught in a pincer attack by Ghost: Sanae Houzuki and Ghost: Asagi Hizuki.

Returning exhausted to the lobby, he witnesses his target Yo Haibara descending in the elevator.
There is no other way, he must find a way to restart the elevator!

Subsequently, he is attacked by Ghost: Michihiko Ikushima and Ghost: Fuyuko Shiratsuki in the lobby.

After defeating the spirits, he enters the passage to the right of the elevator and uses the Long Thin Iron Key to unlock and enter the consultation room.

On the desk in the consultation room to the left, he retrieves the recording Tape 'Consultation Record: Takashi Aiba' and the file Relationship between Lunar Disease and the Moon.
After investigating, exiting through the upper right door leads to the reception desk in the lobby.

Entering the reception area, to the left he sees Ghost: Nurse placing a sign. On the counter where she just stood, he successfully retrieves the Moon Key: Lower Half.

Entering, he immediately sees Ghost: Assistant hiding something inside the window to the left.

As he goes deeper, the phone on the desk rings strangely. After answering, he retrieves the file Assistant's Notes 3.

Entering the curtained room, he obtains the mysterious Rogetsu Craft Box with crescent and lower-crescent patterns on the shelf to the left.

Leaving the medical record room, he walks left to the stairs and first enters the director's office on the left.

Upon entering the office, he sees the residual image of Ghost: Director passing by the door.

On the bookshelf to the left, he retrieves the shocking insider file Director Haibara's Notes 1.

Leaving the office, he goes down to the basement to search for the other half of the key.

Approaching the iron gate on the left, he sees Ghost: Assistant on the other side of the gate.

Reaching through the gate, he successfully retrieves the other half of the key—Moon Key: Upper Half.

Now equipped with both the upper and lower halves, Choshiro inserts them into the Rogetsu Craft Box. With a click, the box opens, revealing the file Note with Coded Clues.

Returning to the office with the note, investigating the intercom on the desk reveals a 3x3 numeric keypad identical to the one on the note.

The note says: "Like a perfect moon."
Choshiro deduces it is a "Magic Square" math puzzle: fill in the numbers 1-9 without repetition so that the sum of each row, column, and diagonal equals 15.

Based on known numbers, he enters the bottom row: '276'.

After pressing the button, a mechanism triggers, activating the file cabinet in the corner.
Pushing aside the heavy cabinet, he discovers a secret passage leading to the basement research lab.

Stepping into the secret passage into the research lab, he is again attacked by Ghost: Katsuto Kariya.

After purifying him, he retrieves the file Lunar Disease Research Record detailing cruel experiments on the table to the right. Continuing to explore, he goes down to the deeper B2F.

Walking to the end, he opens the small wooden door. Beyond the door, an elevator is visible; it turns out the elevator taken by Yo Haibara in the lobby ultimately leads here.

Choshiro, looking at the scene, clearly recalls the moment ten years ago when he discovered this secret space and rescued the spirited-away girls.

At that time, the five girls were in an abnormal mental state, slumped on the floor with vacant eyes, and Ruka kept murmuring an unsettling word...

While searching for Ruka in the infirmary, Choshiro Kirishima accidentally discovered the traces of Yo Haibara, a suspect wanted by the police for local serial murders.

Yo Haibara's father is the powerful Director of Haibara Infirmary, Shigeto Haibara.
Considering that the kidnapped girls were found in a secret room in the basement of this infirmary ten years ago, it is clear that Yo Haibara was also deeply involved in the incident in some form.

Because his whereabouts couldn't be traced at the time, Yo Haibara has remained at large. Choshiro, unwilling to give up, resigned from the police force and became a private investigator to continue tracking Haibara.

And now, the man he has been tracking for so long is hiding in this infirmary filled with spirits.
Perhaps Ruka, whom Sayaka entrusted him to find, is also here...

Choshiro feels that the current situation has too many unsettling similarities to the incident ten years ago. Thus, he grips the Spirit Stone Flashlight and steps once again into the forbidden place where he found the five girls a decade ago.
