Through his investigation of Haibara Infirmary, Choshiro learns that the medical practices inside were not only illegal but deeply inhumane.

For some sinister reason, Director Shigeto Haibara treated the hospitalized patients as mere test subjects, heavily aided by his son, Yo Haibara.

But why?
What drove Shigeto Haibara to be so dangerously obsessed with treating 'Moonlight Syndrome'?

Choshiro tracks Haibara's whereabouts and searches for Ruka through the suffocating air, eventually reaching B2F—the very place where he discovered the five missing girls ten years ago...

The shallow water in front of a small shrine reflects a dim, dark glow. There, he finds Tape 'Choshiro's Investigation Record 4'. Listening to his own voice, memories of rescuing Ruka here years ago surge back like a flood.

Entering the stone door to the right of the small shrine leads into the eerie 'Rogetsu Cavern 1.'
Following the damp passage to the end reveals that the path has been completely blocked by collapsed rocks, leaving a gap too narrow for anyone to pass.

On the ground of this desperate dead end, Choshiro retrieves a terrifying file: Scribbled Suicide Note.

As he turns back, Ghost: Masanobu Fuchiki suddenly ambushes him. Breaking free, Choshiro retreats to the elevator on B2F, only to find Ghost: Satoe Mizue already lying in wait.

Only by defeating them can he successfully take the elevator to escape.

Entering the elevator, it automatically ascends to B1F. The doors slide open, and in the cold corridor to the left, he spots Ghost: Nurse in Front of the Morgue.

Suppressing his fear, Choshiro follows her shadow and pushes open the door into the morgue.

On a table to the left of the entrance lit by a dim candle, he discovers the file Autopsy Report: Tomoko Numata.

Turning his gaze to the front left, the body bag on the medical bed looks unusual... Raising his flashlight, he captures a horrifying image: Bound Ghost: Corpse with Face Peeled Off.

Searching the corner to the right, he finds Notice: Burn Incorrect Documents and the clue image Back Door Key inside a trash bag.
He learns that marked sensitive documents were sent to the incinerator in the old building's backyard to be destroyed.

On another candlit table to the right, he retrieves Tape 'Choshiro's Investigation Record 2'.
On the freezing bed next to it lies the file Autopsy Report: Tsubaki Tohno.

Just as he prepares to push open the door to return to the corridor, Ghost: Masanobu Fuchiki blocks his path once again. After a fierce struggle, he returns to the hallway and spots Ghost: Ward-Rounding Nurse standing before the far left door.
Following her, he steps into a dark underground passage.

Reaching a fork in the path, he sees Ghost: Guiding Nurse. Following her direction to the iron shutter at the end, pressing the switch yields no response.

Photographing it reveals clues of Some Device; it seems the switchboard is located in a deeply gloomy area.

Returning to the fork, he proceeds up the corridor lined with solitary rooms.

The moment he sets foot in the corridor, Ghost: Masako Kitazume, Ghost: Masanobu Fuchiki, and Ghost: Satoe Mizue attack simultaneously, as if all malice has gathered here.

After conquering this horde of spirits, he proceeds to the upper section of the corridor. Finding the switchboard on the far right wall, he flips the switch to restore power.

He then heads to the 'Under-Construction Passage' in the upper right. On a wooden box at the end, he retrieves Construction Worker's Log, uncovering the secret that the B2F elevator shaft overlaps with a portion of the ancient cavern structures.

As he turns to head back, Ghost: Tomoko Tsukiyomi suddenly materializes to attack.

After resolving the fight, he returns to the iron shutter in the passage. Pressing the switch, the shutter rises slowly with a heavy metallic groan.

Passing through the shutter and going upstairs, he sees a ghost enter the director's office on the right. Before entering, he discovers Notice: Special Consultation Schedule on a hospital bed near the doorway.

Entering the room, in the upper right corner, he spots Ghost: Yo Haibara Researching Documents. He seems focused on some forbidden study.

On a small table in front of the desk, he retrieves Letter from Soya Yomotsuki. On the right bookshelf (where the ghost just stood), he finds Director Haibara's Notes 2.

Pushing aside a heavy cabinet in the right corner reveals a hidden path. Following the stairs downward leads into a freezing research lab.

The sharp stench of preservatives fills the air, and beside the medical bed appears the Ghost: Yo Haibara in the Research Lab.

On the bed, he retrieves the file documenting the unusual condition of the girls: Observation Record.

Returning to the director's office and heading back to the corridor, he proceeds to the medical record room. The telephone on the desk suddenly rings with a piercing chime...
On the desk, he picks up the file Assistant's Notes 4.

Continuing forward and unlocking the door to enter the consultation room, the vengeful spirit Sanae Houzuki has been waiting for a long time and launches a fierce attack.

Proceeding to the lobby, his gaze catches the Ghost: Guiding Nurse 2 in the passage next to the stairs.

Following the ghost into the passage, he moves through the surgery room all the way to the record room. Before the doorway on the other side, he sees Ghost: Guiding Nurse 3 signaling the direction.
Using the previously acquired Back Door Key, he opens the door leading out to the gloomy backyard.

The incinerator shed on the right is filled with the heavy scent of ash. Inside the incinerator, he discovers the surviving file Bundle of Letters Narrowly Escaping Fire; near the trash bag to the right, he finds the file About Burned Film Reels.

Stepping out of the shed and heading right, the Ghost: Guiding Nurse 4 manifests in the shadows of the bamboo forest.

Moving along the bamboo path, the Ghost: Guiding Nurse 5 appears once more, as if leading Choshiro toward a specific destination.

Continuing forward, he spots the Ghost: Guiding Nurse 6, at the end of which lies the desolate graveyard.

Behind a cluster of tombstones, he sees the Ghost: Nurse in Front of the Grave standing quietly.

Investigating the gravestone where she stood rewards the file Tsubaki Tohno's Diary and the critical clue item Stained Film Reel.

Now he must locate a projector to uncover the truth hidden inside the film.
On the way back, a vast number of spirits will attack repeatedly; ignore them and sprint back to the hospital at full speed!

Fleeing back exhausted into the prep room, inside the dirty sink ahead, he suddenly catches sight of the Ghost: Man in the Sink.

Taking the stairs up to the second floor, he sees the Ghost: Yo Haibara Heading to the New Building walking toward the connecting passage. Following his footsteps, Choshiro heads to Rogetsu Hall.

Heading left through the elevator lobby into the dining room. Using the projector in the center to play the Stained Film Reel, the footage reveals that during the festival years ago, a maiden suddenly died mysteriously... but what was the actual cause?

After the footage ends, a pale foot unexpectedly appears beneath the projection screen! Suppressing his terror, investigating the fireplace behind the screen rewards the item Burned Painting and Tape 'Choshiro's Investigation Record 3'.

Returning to the elevator lobby, the Ghost: Nurse in the Elevator is standing before the elevator car. Following her inside, he takes it to the third floor.

Stepping out of the elevator, the Ghost: Nurse in Front of the Station appears to the left.
Entering the station where she stood, he retrieves the file Third Floor Nurse Log and Tape 'Choshiro's Investigation Record 5' from the left cabinet.

Proceeding through the other door to the ward corridor, turning the corner leads to Ruka's former room—Room 308, 'Mizuki'. In front of the bed, the Ghost: Sayaka Coming to Retrieve Personal Items is packing up belongings.

On the chair beside the bed, he obtains the clue image Photo Taken in Front of the Hospital and the file Sayaka's Diary 4.

Just as he prepares to leave, Ghost: Shoji Katagiri blocks his path at the doorway. Defeating him rewards a Small Key.

Returning to the corridor and looking right, the Ghost: Man About to Look at Paintings is heading toward the gallery. Following him to the gallery door reveals it is locked.
Left with no choice, he must try to find a path through adjacent Room 309, 'Irodorizuki'.

The moment he steps into the room, he is suddenly tripped! The spirit YoMitsu Magaki (Yuromitsu Magaki) manifests and attacks immediately.

Exerting great effort to defeat him, he investigates the eerie painting in the center of the floor. Memories surface, and Choshiro recalls the day he returned to the island once more in search of Ruka...

The key to everything seems to be the ritual on the island—the Rogetsu Kagura. Because many contracted Moonlight Syndrome due to the ritual, the Haibara father and son believed the Kagura masks and music could aid in treating the disease.

On the day the mass disappearance occurred ten years ago, the Kagura festival was coincidentally being held as well. What does this bizarre coincidence truly imply?
