Room 310: Kamisari
Misaki returns to Room 310, 'Kamisari', her former quarters. Inside this ward layered with echoes, she uncovers an old photograph of her childhood self standing next to a woman whose face is completely blurred.
How are this black-clad girl and that woman intertwined? A fierce intuition whispers that finding this person will unlock every fragmented mystery of her fractured past...
Just as she turns to leave, the doorway suddenly frames the Ghost: Girl in Black Leaving the Room.
On the cabinet holding a record player in the corner, she discovers a photograph of Misaki alongside the black-clad girl, and retrieves the document Misaki Aso's Treatment Plan.
Leaving the room for the elevator lobby, she calls the lift. As the doors open, the Ghost: Girl Standing in the Elevator manifests eerily inside.
Using the Zinc Flat Metal Key, she unlocks the 4F button on the panel and ascends to the fourth floor.
4F: Elevator Lobby
The lift doors slide open. The moment she steps into the 4F lobby, she is brutally ambushed by the vengeful spirit Masako Kitazume.
Nurse's Station: Zanto
Defeating the spirit, she enters the nurse's station. On the left desk, she retrieves 4F Isolation Ward Report 2 and 4F Room Assignment List. Suddenly, a telephone pierces the freezing silence of the room, sending chills down her spine.
Stepping behind the counter, she finds 4F Isolation Ward Report 1 on the table.
Looking up, the Ghost: Girl Operating the Panel appears in front of the wall system.
Investigating the wall panel reveals it is electronically locked. Backing away triggers a wave of spiritual energy; photographing it with the Camera Obscura uncovers the clue image Cipher Echoing in the Ears, revealing the unlock code '4467'.
Re-operating the panel and entering '4467' successfully unlocks the door leading into the corridor.
4F: Isolation Ward Corridor
Passing through the newly unlocked door left of the counter, she enters the corridor and witnesses the Ghost: Two People Walking in the Corridor ahead.
On a small corner table, she picks up 4F Isolation Ward Report 3.
Advancing further toward the main gates of the isolation ward, the Ghost: Blocking Nurse suddenly manifests, immediately followed by an assault from the vengeful spirit Shie Onosaki.
After a fierce battle to subdue her, Misaki unlocks the heavy doors and enters the containment block.
In front of Room 412, 'Hiyuki', she catches the residual sight of the Ghost: Two People About to Open the Door.
On the desk right of the door lies Assistant's Notes 6.

Pushing the door open into Room 412, the Ghost: Woman Holding a Doll appears quietly on the mattress.
On the desk left of the entrance, she retrieves Assistant's Notes 5. Left behind on the bedsheets is the file Sakuya's Diary 1.
Moving to the round table near the far window yields Sakuya's Diary 2. Glimpsing outside the glass, she spots the Ghost: Young Misaki Gazing at the Moon staring intently into the dark night sky.
Circled through the left door into the outdoor greenhouse room, she gathers Misaki's Diary and an Aeria Craft Fragment from the floor.
Returning to the room's corner, she discovers a puzzle box on a chest lined with old dolls. Slotting the Aeria Craft Fragment into place opens the tile puzzle.
Solving it unlocks the box, revealing Sakuya's Diary 3.

At that moment, she sees the Ghost: Staring Girl in Black near the bedside. Approaching her, buried childhood memories surge into her consciousness like an untamed tide.
When her mind clears, Misaki finds herself standing on the open rooftop terrace of Haibara Infirmary.

Near the rooftop railing, she spots the horrific trace of the Ghost: Leaping Patient.
As she draws near, the spirit casts herself off the edge... Investigating the ledge reveals the document Suicidal Woman's Letter on the concrete floor.

Coming to the second floor, Patient's Notes can be found under the mattress on the right side.
Opening the door forward into the hallway leading to the lounge, the Ghost: Man Standing in Front of the Room appears. She enters Ward 1 where he stood.
On the hospital bed in the front-right corner of Ward 1, she retrieves Fragment of an Unsent Letter.
As she prepares to leave, the atmosphere turns heavy, and the vengeful spirits Takashi Aiba and Iori Aiba launch a simultaneous attack.
Defeating them and returning to the hallway, looking left frames the Ghost: Director Calling Misaki to the Lounge.
Moving further forward brings her above the lobby. On the stairs, she spots the Ghost: Director Descending the Stairs.
Following him down, right before reaching 1F, she witnesses the Ghost: Director Heading to the Consultation Room again.
Haibara Infirmary 1F
Standing directly in front of the hospital, pointing the camera upward to the top left corner (near the edge of the grey platform) allows her to capture the Bound Ghost: Woman Falling from the Rooftop.
Returning to the 1F lobby, tracking the path where the Director vanished through the consultation room into the medical record archive reveals the Ghost: Director Moving Deeper Within.
Giving chase down the hall outside the director's office frames the Ghost: Director Standing Outside His Office.
Do not enter the director's office yet. Head down the stairs to the B1F hallway lined with solitary rooms.
Along the lower pathway (turn right after entry), there is a tiny peephole looking inside one of the cells.
Lining up her lens with the hole captures the Bound Ghost: Hanging Woman.
Returning to 1F and entering the director's office officially.
Through a small window near the top right of the chalkboard on the right entry wall, she can capture the Bound Ghost: Eye Peeking from Outside, which seems to have been watching this room all along.
On the small side table before the main desk, she secures Medical Record: Misaki Aso 2.
Operating the intercom panel on the desk triggers the mechanisms of the corner cabinet, sliding it away to reveal the hidden path straight to B2F.

Descending into the B2F research lab, the memory projection of Ghost: Director Haibara and Misaki appears in front of the examination bed. Immediately after, the vengeful spirit Himiko Kiritani attacks in a frenzied rage.
Subduing the spirit, she picks up Medical Record: Misaki Aso 3 from the bed.
She then spots the Ghost: Turning Girl heading down into the tunnels. Following her down the ancient stone steps frames the Ghost: Girl Heading Deeper Inside.
Moving onward, she ducks under a low entry wooden arch into the Moon-Reflecting Cave. Shifting left reveals a residual memory: Ghost: Misaki Being Taken Away.

Reaching the small shrine triggers a massive ghost ambush. Clearing the encounter rewards the document Origin of the Moon-Reflecting Well from the shrine altar.
At this point, Misaki finally remembers being brought here as a child, leaving behind something of ultimate importance before being dragged away by force...
Tsukiyomi-do: Tsukiyomi Path
Regaining her focus, Misaki finds herself standing on the Tsukiyomi Path leading into Tsukiyomi-do, with the Ghost: Girl Heading Onward lighting the path ahead.

Following her into the Resonance Cavern, images flash in her mind of being forced to wear a mask and act as a musician for the 'Kari-me' ritual.

Emerging victorious, Misaki finally remembers that the black-clad girl's name is 'Miu.'
Something of ultimate importance left behind by Misaki
The 'faceless woman' was Sakuya, who once resided in Haibara Infirmary; she had given her own doll to Misaki.
As Moonlight Syndrome worsens, it causes split personalities before ultimate death. To slow her condition's decay, Sakuya treated the doll as herself, holding it tightly to maintain her remaining identity.
That doll was Sakuya's alter ego and at the same time a mirror of Misaki; this is the true identity of the black-clad girl—Miu.
Reaching the subterranean chambers of Tsukiyomi-do, Misaki finally faces Sakuya, but she is no longer the woman who once smiled with gentle warmth; the failed 'Kari-me' ritual on the island caused Sakuya to completely 'bloom.'
She forces anyone who looks upon her to 'bloom' instantly. Misaki is gathered into the embrace of Miu—no, the vengeful spirit of Sakuya—feeling herself fading into a complete, fatal 'bloom.'
